When renewing a passport for a child under 18 years old, Annexure D is one of the mandatory documents that requires notarization and must be submitted along with other required documents.

According to VFS instructions, it’s recommended to type out the information rather than sending handwritten documents. However, I’ve personally seen hundreds of Annexures filled out by hand with pen, and they were accepted.
If both parents are available to sign, mention both parents’ full names on the first line.
If only one parent is signing, please specify the signing parent’s name on the first line.
Additionally, you must submit Annexure C [Only required for Cases where One parent has not given Consent]. Common reasons for one parent’s inability to sign may include travel abroad, separation, or divorce.
Fill out the parent’s full name as stated in the child’s passport (unless there’s a need for correction, which requires two IDs for the new spelling). Next, provide the current USA address including the zip code (ensure it matches the one you’re providing proof for).
Then provide the child’s full name, followed by the father’s full name and mother’s full name.
Child’s full name, followed by the date of birth in Indian format: DD/MM/YYYY (exactly as appears passport), and place of birth same as your passport.
Once you have completed this form, it needs to be notarized. You should wait to be in front of a notary to have your signature witnessed, indicating the place and time of signing this annexure D.
Additionally, provide one of the following for each parent signing, or just one parent if applicable: passport number, Aadhaar number, or voter ID card number.
For notarization, I make it a practice to include the notary certificate wording directly on the same paper. I affix Acknowledgement Stamp or Jurat Stamp, along with my Notary Stamp, and conclude with my signature.
